But for anyone around the world, you can print off the patient information form and mail it to us with your hair sample, and your payment.
We require you to submit the patient health form so Dr. Hull can provide the most accurate nutrition recommendations and interpretations for your analysis. Payment is required before we send your hair to the lab, so make sure you include payment with your health form and your hair, either online or offline via postal methods.
