Where do I mail the form, my hair and payment?
Follow the steps in the hair analysis form to ensure you have completed the form correctly. Instructions are provided within the form explaing how to cut and send your hair sample.
Payment is accepted at the time the hair analysis is submitted to Dr. Hull before the sample is sent to the lab. Payment can be made with a personal check or money order mailed to Dr. Hull with the hair sample and your personal information form.
After you have filled out your information in the hair analysis form, ready to send your hair sample and payment, you can send everything to Dr. Hull to the following address:
